If you would like us to pray for you or someone else, please let us know by filling out the form below. We will add your request to our prayer list and our Pastoral Care Team will keep you in prayer for the next thirty days. 

If you would like one-on-one prayer support with a Unity Prayer Chaplain, please indicate that on the Prayer Request form. Also include your name, phone number and a preferred 2-hour time range, and one of our Prayer Chaplains will call you and pray with you.
